* Permission Slip & Consent Forms due at TMS or mail them to TMS, 50 Norris Road, Tyngsborough, MA,. 01879 ATTN: Camp Bournedale
* First Payment Due ( $125 )
Checks Payable to: Tyngsborough Middle School. Be sure to include child's name on check
* Letters requesting to be a chaperone due
Sept. 4, 2009
* Second Payment due ( $100 )
* Health Forms due.
Must be signed by child's physician. (This does not mean the child needs a physical. The signature is needed if any medications are to be administered while at camp including any over-the-counter medications (e.x.Tylenol))
Sept. 25, 2009
* Third payment due ($60)
This third payment will reflect the added cost of transportation MINUS fundrasiding.
